They empower patients by assisting them in making effective lifestyle changes to control their disease and by providing state-of-the-art treatment, education and monitoring. All of these program components enable patients to live more independently, maintain optimal cardiovascular health and reduce hospitalizations related to their disease.

Meet the Staff:
Lenore Blank, MSN, NP-C
Administrative Manager
Lenore has more than 20 years experience in cardiac nursing, including critical care, home care, and the management of congestive heart failure patients. Prior to her position at Hackensack University Medical Center, Lenore had launched a successful heart failure program at another institution that resulted in an 87% decrease in admissions to the hospital for these patients. For these efforts, she received a nursing recognition award in 1998 from the New Jersey League for Nursing.

Michele Gilbert, MSN, NP-C,CCRN, CNN
Education Coordinator
“Mickie” has a diverse clinical background that includes more than 25 years in critical care, nephrology, home care, and, most recently, the care and management of heart failure patients. As Education Coordinator, she is responsible for teaching patients and staff members about heart failure management strategies. Her educational efforts are conducted both on site at HUMC and in the homes of discharged patients through the Home Health Agency. Mickie also writes many of the educational materials that are distributed to patients.
