Who We Are

Since 1966, Hackensack University Medical Center’s Home Health Agency has provided skilled nursing care, rehabilitation therapy, social work services, and personal care to thousands of patients of all ages in their homes.

Because medical technology and the delivery of healthcare services have evolved over the years, more and more patients are now able to receive state-of-the-art care within the comfort of their homes. From infants to mature adults, patients receive high-quality care to help them improve their functioning, recuperate from an illness, or manage a long-term condition.

The Home Health Agency emphasizes a team approach. Working in partnership with a patient's own physician, the Home Health Agency’s fully credentialed and licensed team includes registered nurses, clinical nurse specialists, physical, occupational, and speech therapists, social workers, and home health aides.

The Home Health Agency is committed to providing quality care. The agency maintains an ongoing documented quality improvement and program evaluation service to further enhance patient care and services by addressing any questions and making regular assessments of its services. Regular staff evaluations are also made by supervising personnel. The agency is licensed by the New Jersey Department of Health and Senior Services, approved by Medicare and Medicaid, and accredited by the Joint Commission on the Accreditation of Healthcare Organizations.
